Dimples are perhaps the cutest defect to happen. Natural Dimples are actually defects in the cheek muscle of the face so that the buccinator muscle separates when it is contracted. Those who are not genetically gifted with dimples can have them with a small dimple surgery procedure within an hour. The surgery is performed under local anesthesia and the patient goes back home the same day.

What is Dimple Creation Surgery?

Dimple surgery is plastic surgery of the face. It is done to create cheek dimples. It is usually done as an outpatient procedure under local anesthesia.

The patient can go home after a few hours. Although the procedure takes only 30 minutes we like to observe the patient for at least 2 hours after that.

What Happens in Dimple Creation Surgery?

Before taking you to the operating room to create dimples the plastic surgeon first marks the correct anatomical position of the dimples on your face in front of a mirror.

You are then asked to choose the position of dimples you like the best.

The procedure is performed under local anesthesia by completely numbing the area. There is no pain during the procedure.

A small incision is placed inside the mouth to release the facial muscles called buccinators. I.e a small portion of the muscle is separated from its attachments to the overlying skin as well as the mucosa inside the mouth.

An absorbable suture is then placed across the cheek bringing the two surfaces, skin and mucosa, together in between the separated muscle. The knot is tied inside the mouth. There are no incisions or knots on the skin outside.

This suture is left in place for it to dissolve on its own. No dressing is required.

Cold packs are applied for twenty minutes to reduce post-operative swelling.

So I want to get Dimple Surgery. How do I proceed?

The first step always is to book an appointment and consult with the cosmetic surgeon.

Your plastic surgeon will perform an in-depth analysis of your face and ask you some relevant questions. This is precisely the time when you should discuss what you want and what are you apprehensive about. Don’t be shy. All questions are important. You should be aware of all the pros and cons.

The surgeon then proceeds to mark dimples on your face and emulate the dimpling using earbuds to show you in front of a mirror.

After making up your mind you need to fix a date for the surgery.

You are asked to come to the surgical center at least 2 hours before the time slot allotted for your surgery.

It is essential to come on an empty stomach if your procedure is planned under sedation or GA. Please carefully read and follow all the pre-operative instructions given to you at the time of appointment.

You will be given time immediately before the surgery to discuss any last-minute doubts with the surgeon. The surgeon, along with you, will mark the location of your new dimples with an aesthetic analysis in front of a mirror. Once you are ready, you will be taken to the operation theater.

What are the Post Op care instructions and precautions for the dimple creation procedure?

You will be explained and given a printed postoperative instruction sheet. Please read that carefully and completely. Follow all instructions sincerely.

After the surgery, there will be a slight indentation of the suture on the cheek. The suture will be palpable below the skin for 2 weeks. Proper skin care for the first few weeks includes a daily wash with soap and water and application of topical antibiotic ointment.

Similarly, a few sutures will be palpable to the tongue inside the mouth. For the first two days, you are allowed to drink clear liquids, juices, etc. Milk-based products are to be avoided.

It is preferable that you consume cold liquids as hot drinks exaggerate swelling.

Chewing should be avoided for the first two days.

Adequate oral hygiene is essential. Antiseptic Mouth wash liquids are used for a week after the procedure.

From the third day onwards easily chewable semisolid diet is preferred.

Smoking and alcohol consumption is restricted for a month.

When will I see the results of my Dimple Creation Surgery?

The Absorbable sutures need not be removed. You will appreciate the result immediately.

There will be mild swelling and redness for a week or two. Tissue healing takes 1 to 2 weeks. Initially, the dimples will be very deep on both sides even without smiling.

Gradually over the next couple of weeks, the static dimples(resting face) will become less noticeable with just a slight dip in the cheek.

Complete tissue healing and remodeling takes around 4-6 months.

What Are the risks and safety considerations ?

Dimple creation is a safe surgery. Like all surgical procedures, there are risks related to surgery, anesthesia, and sutures. Please read the Dimple Creation Informed Consent document thoroughly and completely to understand the risks involved.

Side effects may be grouped under two types :

Immediate post-operative side effects

  • Pain – there will be slight pain which is easily managed by pain medications.
  • Swelling – There will be swelling for the first few days.
  • Redness – redness for the first few days is normal. But Throbbing pain and redness past the first week may be signs of infection. In this case, urgent consultation with the surgeon is required.
  • Hematoma, blood clot – Ordinarily there is minimal bleeding during the procedure. Sometimes there may be blood clot formation which will need to be surgically evacuated.
  • Suture line infection – although rare seen it is easily controlled by oral antibiotics for a week.

Long term side effects include

Less than the desired depth of dimples, Uneven level, Too high or too low displacement. The precise location of the cheek dimple also depends on the facial anatomy. There is no correct position of the dimple. The exact location of naturally occurring dimples may vary depending on the individual anatomy.

So slight uneven level may not be because of surgical failure. The muscle anatomy will also decide the precise location.

In case the levels are markedly different a correction procedure is performed after 6 months from the day of surgery.

Who are the best candidates for Dimple creation ?

The simplicity of the procedure makes it conveniently accessible for everyone. There is no age limit though we defer patients younger than 18 years. It is preferable for the patient to be in good physical health and most importantly must have a realistic expectation. It’s important to understand that any cosmetic surgery can only change your physical appearance and not deliver a social impact.
